We are active clinicians, policy makers researchers and epidemiologists who research, teach and provide clinical care to patients. We seek to develop new ways of providing care to patients and we are developing an academic primary care centre in a deprived area that will act as a national and international exemplar.

The University of Kansas Department of Family Medicine has converted to a patient centered medical home and has applied of recognition. We use an electronic record platform supported by Epic which is used by all other departments in the University and in the Kansas University Hospital. We have recently added an interactive patient portal and are working with the hospital to provide chronic care management of CHF and COPD. We are using the hot-spotting model for management of at-risk populations.
The Family Center Experience (FCE) & Longitudinal Case Studies (LCs) is a required course in the University of Michigan Medical School curriculum. Through FCE, first- and second-year students engage in understanding the personal side of medicine through first-hand encounters with patients and their families. This knowledge base is expanded with the LCs, which are designed to empower students to explore the biomedical and sociocultural impact of disease through self-directed and small group learning.
