This program is an in-depth guide to NCQA Recognition requirements and the process for achieving PCMH recognition. Recognized practices and clinicians have proved the relevance of the NCQA PCMH standards to the Triple Aim. The 2017 standards emphasize a practice wide commitment to sustaining the PCMH transformation, demonstrating patient care and population health management, care coordination and ongoing quality improvement.

Practices are rewarded for integrating behavioral healthcare and addressing the impact of social determinants on patient needs. NCQA requirements are positioned to qualify for the Merit-Based Incentive Performance System (MIPS) Clinical Practice Improvement Activities (CPIA) and to support other MIPS requirements under MACRA.

Additionally, a special discussion will be held for New York State attendees and others interested in learning about the New York State Patient-Centered Medical Home (NYS PCMH) Recognition Program.
