NCQA is planning an ambitious redesign of the PCMH recognition program in response to feedback from clinicians, practices, employers, payers, health services researchers and other key stakeholders.

Loreta Villemez

Loreta is the current Team Champion for the PCMH model of care. Loreta is also a key player in maximizing potential from HEDIS and pay-for-performance opportunities. She practices her hidden agenda (as much as is permitted) to integrate Lean Six Sigma principles in the development of new processes and in improvement activities. Most recently, Loreta co-led a team that helped Neighborhood Healthcare achieve PCMH Level 3 Recognition for 7 primary care sites previously recognized at Levels 1 and 2. read more

Peggy Vazquez

Peggy has over 35 years of leadership achievement in the health care field. Retired from U.S. Army Nurse Corps where first experienced true integrated medical care and joined Group Health Associates, a multi-specialty physician group who pioneered Healthcare Quality in Cincinnati, Ohio. While there studied the teachings of Dr. Don Berwick (Institute of Healthcare Improvement). Inspired by Crossing the Quality Chasm Building the 21st Century Health Care System and embraced the importance of Patient Centered Medicine. Joined Primary Health Solutions Sr. Leadership in 2009, and remains one of her most rewarding career opportunities. read more

Joe Grundy

Joe Grundy is responsible for supporting primary care practices, and clinical teams, through quality improvement initiatives to achieve truly patient centered care across the Saint Luke’s Medical Group. This includes leading practice transformations to continue implementing the patient-centered medical home model. Before coming to Saint Luke’s Joe served as the Delivery System Strategist for the American Academy of Family Physicians wherein he monitored and helped foster disruptive new delivery models for family medicine. read more

Michael S. Barr, MD, MBA, MACP

Executive Vice President - Quality Measurement & Research Group, NCQA

At NCQA, Dr. Barr works on tackling big issues in our health care system – promoting delivery system reform and leading NCQA’s performance measurement, research, analysis and consulting work. Prior to joining NCQA, Dr. Barr was Senior Vice President of the Division of Medical Practice at the American College of Physicians (ACP), where he helped lead advocacy for the PCMH model and practice transformation. Dr. Barr thinks that among the changes needed to strengthen our health care system, two key needs include reducing the burden of non-value-added work required of health care professionals and changing the payment model from fee-for-service to one that aligns with the goals of the triple aim. read more
