The Transforming Clinical Practice Initiative (TCPI) grant awards, announced by the U.S. Department of Health and Human Services last month, aims to equip more than 140,000 clinicians with the tools and support needed to improve quality of care, increase patients’ access to information, and reduce costs. TCPI is one of the largest federal investments designed to support ambulatory care teams in all 50 states through collaborative and peer-based learning networks over the next four years (Practice Transformation Networks, or PTNs, and Support and Alignment Networks, or SANs).
This webinar will provide an overview of the PCPCC's SAN grant, including its four main goals, metrics, and the roles of our partner organizations. Titled "Patient, Caregiver, & Community Engagement Support & Alignment Network (PaCCE SAN)", the PCPCC will collaborate with the Institute for Patient- and Family-Centered Care, Planetree, and the YMCA of the USA to carry out this work.
