Change of shift report is the time when responsibility and accountability for the care of a patient is transferred from one nurse to another. Bringing nurse change of shift report to the bedside is integral to enhancing patient safety. Yet, discussion of the patient with the patient and/or family present raises several challenges for nursing staff. Long-term services and supports (LTSS) are a vital part of care for many dually eligible beneficiaries. Individuals from racial and ethnic minority groups often experience disparities in access, quality, and outcomes in LTSS, such as higher rates of rehospitalization under home health care[1] and lower quality of care in nursing homes[2].

This helpful webinar will provide best practices, tools and approaches used by staff liaisons to build successful patient- and family-centered programs. It will also highlight key components of well-established advisory programs and effective strategies utilized by individuals in this role to build partnerships between leaders, staff and clinicians, and diverse advisors.
The webinar will discuss the concept of primary care spend and resources that are available to chapters. The webinar agenda is listed below:
1) Dr. Evan Saulino, Oregon AFP, will discuss how the Oregon AFP was able to advocate for primary care spend reporting requirements and level mandates.
2) Andrew Bazemore, Director of the Robert Graham Center, will discuss primary care spend research.
Advocating for oral health for children doesn’t have to be hard, especially if you are already advocating for other health issues. If you haven’t had too many discussions lately without your teeth, you understand why oral health is everywhere and why it is often forgotten or put to the side.

Evan Saulino, a family physician at the Providence Southeast Family Medicine Clinic, will describe innovative work by the Oregon Health Authority Patient-Centered Primary Care Home Program (PCPCH). Dr. Saulino acts as the Clinical Advisor for Oregon’s PCPCH Program, which works with clinics serving over 3 million patients across Oregon.
