It's an all-too-familiar scenario: a doctor mapping out next steps at a medical appointment — perhaps prescribing new medications or ordering some tests —oblivious to the costs, including whether the patient might bear the brunt of them. Some now consider this a form of harm that physicians, nurses, and others need to become more conscious of and do everything possible to prevent.
Management of chronic care needs, including care coordination between primary and specialty care clinicians, has received much attention lately, especially from legislators and federal regulators where Medicare beneficiaries are concerned. Specifically, proposals on the sustainable growth rate issue call for creation of a payment code for care management for individuals with chronic conditions, and the Centers for Medicare & Medicaid Services (CMS) wishes to establish in 2015 additional payment codes for chronic care management services.

The "Open Forum Calls" provide leadership the opportunity to report out to the public on recent and upcoming center activities, encourage open discussion, and solicit public feedback on center work. These calls may also feature guest speakers who will present on timely issues pertaining to advanced primary care and the patient-centered medical home.
