The 2014 standards emphasizes team-based care with a significant focus on the care management of high-need populations along with the integration of behavioral health. We have aligned the Quality Improvement (QI) activities with the Triple Aim and positioned the NCQA requirements to qualify for Meaningful Use Stage 2.
