PFE Bite-Sized Learning Series

In partnership with X4Health, the PCPCC Support & Alignment Network (SAN) is happy to present a series of videos that will quickly guide practices and quality improvement teams to achieve their PFE and QPP goals. This "starter track" will help create a foundational level of knowledge about PFE engagement and implementation strategies for TCPI participants and others working towards practice tranformation.

  • Learning modules take less than 10 mins to complete, and the series reflects feedback we've received from practice coaches & clinicians in the field!
  • Our strategy was to provide you with a creative resource you could complete on your own time- no webinars, no scheduling needed- so that we can spend time helping you apply the learning in more focused ways (e.g., small group projects focused on TCPI Aims, helping a practice or PTN apply PFE to a specific improvement project, helping a PTN plan a PFE integration strategy, etc.).
  • "Lock-in" your learning by answering a few questions and sharing your thoughts about PFE in health care.
