Select TCPI Support and Alignment Network Resources To Enhance PFE in Choosing Wisely

This is a selection of resources offered by TCPI Support and Alignment Networks. The materials assist Practice Transformation Networks and clinicians in TCPI as they adopt Choosing Wisely programs that incorporate meaningful person and family engagement approaches.

  • American College of Emergency Medicine Support and Alignment Network: The ACEP Reduce Avoidable Imaging Initiative includes an Avoidable Imaging Initiative Tool Kit with materials to support patient and family engagement
  • American College of Physicians Medicine Support and Alignment Network: ACP's High Value Care Cases for Adults and Pediatric patients (with integrated CW content) are an offering to all enrolled clinicians in TCPI. ACP SAN: “PFE –THE BETTER WAY TO BE” – is A Coaching/Webinar Series by the Person and Family Engagement Experts. Registration is free to PTN-enrolled clinicians, and PTN and SAN staff.   • American College of Radiology Medicine Support and Alignment Network: ACR Patient- and Family-Centered Care Resources collected by the Commission on Patient- and Family-Centered Care to enhance understanding of—and participation in—new practice and payment models and help clinicians provide more patient- and family-centered care.
  • Patient Centered Primary Care Collaborative: PCPCC’s PFE Resource Center includes information on person and family engagement, resources to help clinicians address the TCPI PFE metrics, recorded webinars and downloadable handout materials for clinicians and patients. The Resource Center will be updated regularly with Choosing Wisely information.
