PCPCC is Now … Primary Care Collaborative!

The Patient-Centered Primary Care Collaborative, a leading health care advocacy organization, is now the Primary Care Collaborative (PCC).

The logo suggests an individual at the center of cascading circles, reflecting PCC’s ongoing commitment to person- and family-centered care.

This logo evolves the organization’s brands by adapting an icon we’ve used for several years to visualize the patient-centered principle of primary care.

The new tagline — Convening + Uniting + Transforming — emphasizes PCC’s unique role in gathering and uniting diverse stakeholders around research, care delivery and payment models, and policies.

“Since launching in 2006, the organization has earned a strong reputation for transforming health care delivery, and we felt it was time that the name and visual identity reflected this reality. As health care increasingly recognizes the value of person-centered, team-based, community-aligned primary care, this new brand embraces our role in shaping the future.”

- Ann Greiner, PCC President and CEO

PCC unveiled the change on November 4 at its annual fall conference with physician and nursing groups, patient and consumer organizations, health plans, employers, quality organizations, health systems, and others.
