The patient-centered medical home (PCMH) continues to help healthcare organizations achieve lower costs and better care, as well as improve physician and staff morale. With such potential benefits, FiercePracticeManagement reached out to Montefiore Medical Group in the Bronx, N.Y., to hear how it has established a PCMH model and received National Committee for Quality Assurance (NCQA) recognition.
For providers looking to transition to the PCMH model, don't reinvent the wheel and recognize it will take time, according to Sabine Nicoleau, director of Montefiore's Patient-Centered Medical Home project.
"Some people think they have to start from scratch but there are a lot of interesting models you can build off of and then tweak to modify to your practice or site," she told FiercePracticeManagement in an exclusive interview. "Also, just be aware that it's a change process; it takes several years to truly transform a practice."
