Lesley Reeder

Lesley Reeder, RN, BSN, is the Vice-President of Nursing Services and Excellence, for InnovAge, a family of non-profit organizations that provides home care, palliative care, assisted living services, and administers Program of All-inclusive Care for the Elderly (PACE) programs in Colorado, New Mexico and soon, in California.Ms. Reeder is a cabinet member of the Primary Care Collaborative’s (PCPCC) Center for Policy and Advocacy. Topics of expertise include: Medicaid participation in multi-payer medical home pilots, developing data-driven care management and care coordination models, payment reform, utilizing stakeholder groups to inform policy-making and practice transformation informed by patient-centered medical home concepts.Previously, Ms. Reeder worked with Rocky Mountain Health Plans managing their Medicaid product lines and before that, she worked in Quality Improvement and Long Term Care for the Colorado Department of Health Care Policy and Financing, the state Medicaid agency. She was also a labor and delivery nurse at Avista Adventist Hospital in Louisville, Colorado and a clinical nurse instructor for Regis University in Denver.
