The Broadlawns Medical Home Clerkship is an elective rotation for fourth-year students at the Des Moines University College of Osteopathic Medicine that offers structured clinical and coordinated care experience under direct supervision at the Broadlawns Family Health Center. Students gain a basic understanding of the various aspects of a medical home as it pertains to enhanced patient care in the primary care setting.
The clerkship has three areas of focus: (1) didactics, (2) clinical processes, and (3) projects. The program’s didactics focus on PCMH healthy policy, chronic disease management, chronic disease registries, and quality improvement projects. Clinical processes focus on pre-visit planning, hospitalization readmission reduction, referral tracking, patient reminders, and provider feedback. For the project, students choose a medical home topic to evaluate current practice methods and propose an improvement plan, which they will then implement for evaluation using the Plan-Do-Study-Act (PDSA) model.
