The purpose is to provide information and education regarding the patient-centered medical home. There are three tracks: Track 1 for anyone wanting information on PCMH, Track 2 and Track 3 are for providers and staff already on the PCMH path. This live activity, Kansas PCMH Summit 2012: Connecting the Dots, with a beginning date of September 14, 2012, has been reviewed and is acceptable for up to 8.25 Prescribed credit(s) by the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity.The Kansas Patient Centered Medical Home Initiative (Ks PCMHI is a project of the Kansas Academy of Family Physicians in conjunction with the Kansas Association of Osteopathic Medicine, the Kansas Chapter of American Academy of Pediatrics, the Kansas Chapter of the American College of Physicians, and the Kansas Medical Society.The Ks PCMHI provides education and information regarding the Patient Centered Medical Home (PCMH) and encourages practices to move to the PCMH model thereby improving population health and clinical outcomes.
