Sponsored by the Care Delivery and Integration Center, the webinar will feature Richard Baron, MD, MACP, Group Director, Seamless Care Models, CMS Innovation Center; Jacob Reider, MD, Acting Chief Medical Officer, ONC; Mat Kendall, Director of Office Provider Adoption Support, ONC; and Johann Chanin, Director in Product Development, NCQA. Dr. Baron will provide a perspective on the role of HIT in seamless care models, including comprehensive primary care, as well as its importance in advancing the capacity for primary care providers in the future. He will focus particularly on the relationship between HIT use and payment reform models and how new payment models will drive – and demand - different uses of HIT. Mat Kendall will speak about Regional Extension Centers (RECs) and their work with primary care providers on their path to meaningful use. Dr. Reider, who is also a family physician, will describe practice transformation through health information technology from a provider's perspective. Finally, Ms. Chanin will discuss the alignment between the patient-centered medical home and meaningful use and how the meaningful use requirements have been integrated into NCQA’s PCMH 2011 standards.
