Medical Director of ACO Programs, Caravan Health

Fred Wallisch, MD is Caravan Health’s Medical Director of ACO programs. In this role he is responsible for clinical oversight of all ACO activities including collaboration with physician leadership on data driven, value-based care strategies to improve patient outcomes, quality of care and utilization metrics to drive client success. He leads the clinical implementation of new and innovative strategies across all of Caravan’s ACOs. Dr. Wallisch provides clinical leadership for all physicians, clinical expertise, and business direction in support of medical care management programs through participation in clinical team activities, policy development, and program performance monitoring. read more

Jose Pena, MD

CEO and CMO, RGV ACO Health Providers, LLC

Jose Pena, MD, FACP, FACHE, is the chief executive officer and chief medical director of the Rio Grande Valley Accountable Care Organization Health Providers, LLC. His primary areas of focus include process improvement and innovations in patient-centered diabetes management. His organization has become one of the most successful ACOs – as measured by both quality metrics and cost savings. Prior to working with RGV ACO, Dr. Pena served in a variety of leadership roles in local hospitals. He is board certified in internal medicine, hospice and palliative medicine. Dr. Pena completed his post-graduate training in internal medicine at St. Barnabas Hospital in New York. read more

Sarah Mullins, MD

Senior Medical Director, Aledade

Dr Sarah Mullins trained at Duke University, University of Maryland Medical School and served as Chief Resident at Christiana Care Family Medicine Residency. She is a practicing family physician and owner of Stoney Batter Family Medicine in Wilmington, Delaware. Her practice joined Aledade as a partner practice in 2014, and participates in value-based contracts through Aledade for Medicaid, commercial insurance, Medicare and Medicare Advantage. Dr Mullins is a Senior Regional Medical Director for Delaware, Maryland and Washington DC, encompassing the 3 ACOs and the MDPCP Care Transformation Organization in these areas. read more
