Jay W. Lee, MD, MPH, FAAFP, is Director of Primary Care at CareMore Health, where he leads the development and implementation of a next-generation primary care model across all markets and continues to serve his medically and socially complex panel of patients in Long Beach, CA.
A graduate of the Program in Human Biology at Stanford University, he worked for a non-governmental organization in post-war rural northern El Salvador supporting local physicians, organizing public health projects, and growing his hair long before returning stateside for medical school at the University of Southern California and family medicine residency training at Long Beach Memorial Medical Center. He then worked at community health centers in southern California and Boston, where he earned his Masters of Public Health at Harvard University with an emphasis in Health Policy and Management.
Before joining CareMore Health, Dr. Lee served as Chief Medical Officer at Venice Family Clinic, a federally-qualified health center serving over 26,000 vulnerable patients in Los Angeles, CA. He has also served as Associate Medical Director of Practice Transformation at MemorialCare Medical Group, where he led implementation of the patient-centered medical home model in practice sites from Long Beach to San Clemente, and as Director of Health Policy and Assistant Program Director at the Long Beach Memorial Family Medicine Residency Program.
