Description: 

Mercy Health Network (MHN) Critical Access Hospitals (CAH), in partnership with Catholic Health Iniativies, is using the CMS Health Care Innovation award to test a model to transition a network of rural critical access hospitals to value-based care. 

This will position CAH facilities so they can participate in value-based care and become involved in the care redesign process. New resources will be available for coordination of care and the care of chronic disease to best meet the specific needs of the patients. CAH facilities will also be able to benefit from shared savings generated by working with an ACO.

The award is $10,171,220 over three years. 

Fewer ED / Hospital Visits: 

Reduced acute inpatient days from 2,274,881 in 2017 to 2,176,954 in 2018*

Reduced outpatient emergency visits from 488,821 in 2017 to 464,717 in 2018*

Increased Preventive Services: 

Increased physician visits from 10,540,482 in 2017 to 10,949,019 in 2018*

Other Outcomes:
