Don't get caught outside the paradigm shift of medical transformation!  Come hear what national experts at NCQA, Blue Cross Blue Shield South Carolina, The Patient Centered Primary Care Collaborative (PCPCC) and other practitioners are doing to transcend and put patients first and how NCQA's 2014 Standards will affect members in the Neighborhood, hear from policy makers, clinical integration experts, behavioral health, pharmacy, social services and faith-based organizations.

On behalf of Dr. Kashyup Patel and Dr. E. Michael Eziri we are inviting the Patient Centered Primary Care Collaborative to join us for the 2014 Carolinas Medical Neighborhood Forum, sponsored by The Medi Awards (www.themedi.org ) being held at The Ballantyne Hotel and Lodge, Charlotte, NC on June 20-21, 2014.

The Carolinas Medical Neighborhood Forum is the first event in a series to bring together leading authorities and practitioners in the field of healthcare transformation.  The first day of the two-day event will consist of experts in the area of Patient-Centered Care and the Medical Neighborhood, from organizations like the National Committee for Quality Assurance (NCQA), Blue Cross Blue Shield South Carolina and the Patient Centered Primary Care Collaborative (PCPCC).  On the second day, we will host the Inaugural Carolinas Medical Neighborhood Golf Tournament with an Awards Luncheon.  There will be several opportunities for you to participate as an Exhibitor and or Sponsor during the two-day event.
