MissionPoint Health Partners was founded by Saint Thomas Health in Nashville, Tenn. which is owned and operated by Ascension Health, to not only serve the healthcare needs of our community but also the poor and vulnerable among us. MissionPoint directly designs and contracts provider networks, deploys health partners who call on patients in the hospital, the emergency room, in doctors' offices or in their homes, and further supports any additional needs members have to live well and flourish in the community.
Founded in 1908, St. Barnabas Senior Services (SBSS) is a dynamic nonsectarian senior service agency in Los Angeles, CA that is dedicated to ensuring that older adults have the right to age with dignity. SBSS provides comprehensive nutrition, healthcare, education, social service programs and special activities designed more than 12,000 older adults each year. SBSS is unwavering in its mission to provide a continuum of innovative services that empower a diverse community of elders to live well, feel well and age well.
Southern Prairie Community Care is an Integrated Health Partnership formed twelve Southwest Minnesota Counties. Southern Prairie Community Care is an Accountable Care Organization focused on improving the quality of care, improving patient satisfaction, and saving costs for Medicaid and Minnesota Care Patients. Our partners include primary care providers, hospitals, mental health centers, pharmacists, county social services, and public health providers. Our Community Care Teams are providing fully integrated care and services for the most medically complex and difficult to engage patients.
