Wyoming was awarded a Center for Medicare and Medicaid Services (CMS) CHIPRA Demonstration Grant. Maryland, Georgia and Wyoming submitted a joint application to implement and/or expand a Care Management Entity (CME) provider model using High Fidelity Wraparound and Intensive Care Coordination. Wyoming seeks to improve clinical, functional, and cost outcomes, access to home and community-based services, and youth and family resiliency of Medicaid children and youth with serious behavioral health challenges and historically high costs or at risk of high cost through implementation of a CME pilot in the Southeastern Region of Wyoming. The primary population for the CME is Medicaid financially eligible children and youth, including:
Children and youth with serious emotional disorders, including a medical or educational diagnosis of serious emotional disturbance/emotional disturbance; or
Children and youth at risk of out-of-home placement and/or children who currently meet Psychiatric Residential Treatment Facility (PRTF) level of care; and/or
Children identified by WDH whose use of prescription drugs do not meet prescribing guidelines established by WDH.
During the 5 year grant period, FY2010 – FY2015, Wyoming’s Grant Amount is about $450,000 per year. While Maryland manages the Grant, a local multi-agency Governor’s Planning Team for At Risk Youth and Families Committee (PTAC) through the CHIPRA Steering Committee manages and coordinates the local effort.
In addition to funding for additional outreach, CHIPRA provides bonus payments to encourage states to enroll children who are already eligible for Medicaid and CHIP.
