Kickoff Learning Collaboratives with Health Care Organizations in Seven Communities
Health care organizations in seven U.S. communities this month begin their transformation to a Patient-Centered Medical Neighborhood (PCMN) concept of care at kickoff learning collaboratives led by TransforMED, a nonprofit subsidiary of the American Academy of Family Physicians.The seven organizations are the first of 15 health systems nationwide to initiate TransforMED training as part of the CMS Center for Medicare and Medicaid Innovation’s PCMN demonstration project. We will review the challenging standards, identify acceptable documentation for the elements and learn how these standards directly link to improved patient outcomes. Identify the meaningful use (MU) criteria and how practices can receive credit for their MU achievements through the PCMH 2011 Standards.

Wednesday, Feb. 27, 2-3:30 PM ET. Learn how to analyze your organization’s processes and prepare your corporate tool. Get tips on how to select appropriate documentation demonstrating common procedural practices across all sites. Identify what to expect during a multi-site review. Upon completion of this session, participants will be able to: Determine eligibility for a multi-site applicant. Ascertain which elements documentation is required from all sites. Submit and manage documentation for a multi-site applicant.
The state of Arkansas' health system transformation is getting a financial boost from Wal-Mart Stores, which committed to providing $670,000 to help underwrite the work of the Arkansas Health Care Payment Improvement Initiative. The initiative, Arkansas Surgeon General Dr. Joe Thompson said, is a multipayer effort, and the intent is to make it an all-payer effort, including self-insured employers. Wal-Mart, based in Bentonville, is the biggest self-insured employer in the state.
The Primary Care Collaborative is criticizing an Annals of Internal Medicine report that concluded there wasn't enough evidence to determine the patient-centered medical-home practice model's effect on clinical outcomes and costs. The collaborative asserts that the report overlooks more recent studies that "tell a far more encouraging story."The journal study, funded by the HHS' Agency for Healthcare Research and Quality, identified some 5,731 studies for analysis, but its authors found only 19 that contained the effectiveness data the researchers sought.
