MMCH is in Batesville, Indiana, conveniently located off I-74 between Cincinnati, Ohio and Indianapolis. MMCH serves a population of more than 65,000 residents in Ripley, Franklin and surrounding counties. The hospital provides a broad scope of services including 24-hour emergency, family-centered maternity, outpatient rehabilitation, home health, hospice and oncology.
Southwest Pediatrics is a primary-care, pediatric clinic located in New Berlin, Wisconsin. The practice is made up of six physicians and thirty support staff members. Over the past eight years the clinic has been working to create a patient centered medical home. To do this a parent advisory board was started, several quality improvement programs were initiated and parent informational meetings and a quarterly newsletter were developed.
Psychological HealthCare is a multidisciplinary group of behavioral health providers located in Central New York. We have 60 providers in four locations in Syracuse and another small office in the Utica region. We work closely with primary care physicians and are interested in integrating treatment more closely with medical offices.
