A strategic priority for the Ohio Department of Health (ODH) is to increase the expansion of the Patient-Centered Medical Home (PCMH) throughout Ohio. The ODH is leading a statewide expansion of the PCMH in order to 1) control costs and ensure healthcare in Ohio is affordable, 2) improve health outcomes and 3) enhance the patient experience. The Patient Centered Medical Home Education Pilot Project was authorized by HB 198 of the 128th Ohio General Assembly. ODH’s first major step in implementing this pilot program was the establishment of the Ohio Patient-Centered Primary Care Collaborative (OPCPCC). The OPCPCC is a coalition of primary care providers, insurers, employers, consumer advocates, government officials and public health professionals who came together to support a more effective and efficient model of healthcare delivery in Ohio. Care in a PCMH is foundational to all of Ohio's health improvement initiatives including the Comprehensive Primary Care (CPC) initiative, State Innovation grant, dual eligibles program, and health homes program.

CMS State Innovation Model (SIM) Test Award - Ohio

On December 16, 2014 Ohio was awarded a $75 million four-year State Innovation Models (SIM) testing grant to implement enhanced primary care and value-based episode payment models in partnership with Ohio's largest commercial health insurers. The state composed a State Health Care Innovation Plan to create a sustainable, patient-centered delivery and payment system over the next three to five years.

Pediatric Ambulatory Care Program - Rainbow Babies and Children's Hospital

University Hospitals (UH) Rainbow Babies and Children’s Hospital at UH Case Medical Center is receiving an award to improve care for approximately 65,000 children with Medicaid with high rates of emergency room (ER) visits, complex chronic conditions, and significant behavioral health problems in several counties across northeastern Ohio.

Ohio PCMH Education Pilot Project

The Patient Centered Medical Home (PCMH) Education Advisory Group (EAG) and the Patient-centered Medical Home Education Pilot Program were created by Ohio’s medical home statute (HB 198 of the 128th Ohio General Assembly). The Ohio Department of Health oversees implementation of the pilot program and seeks funding to support and sustain the initiative, with the assistance and advice of the EAG. The design of this pilot is unique because of its focus on education and advancing medical and nursing education in the PCMH model of care delivery.

Ohio & Kentucky Comprehensive Primary Care Initiative

Ohio’s Cincinnati-Dayton region is one of the seven markets selected to participate in the CPC initiative. The Ohio-Kentucky designated area contains 75 primary care practices, 61 of which are located in Ohio, with 261 providers and an estimated 44,500 Medicare beneficiaries.
