Hoangmai H. Pham, MD, MPH is a general internist and Director of ACO Programs at the Innovation Center within the Centers for Medicare & Medicaid Services. Before joining CMS, Dr. Pham was a senior health researcher and co-director for research at the Center for Studying Health System Change in Washington. She has published extensively in the areas of care coordination, quality performance, provider market trends, health disparities, and the intersection between each of these areas and provider payment policy. She has practiced primary care for many years at a safety net clinic in Washington. Mark McClellan, MD, PhD, is a senior fellow and director of the Health Care Innovation and Value Initiative at the Brookings Institution. Within Brookings, his work focuses on promoting quality and value in patient centered health care. A doctor and economist by training, he also has a highly distinguished record in public service and in academic research. Dr. McClellan is a former administrator of the Centers for Medicare & Medicaid Services (CMS) and former commissioner of the U.S. Food and Drug Administration (FDA), where he developed and implemented major reforms in health policy. Pauline Lapin is the Director for the Seamless Care Models Group (SCMG) in the Center for Medicare and Medicaid Innovation at the Centers for Medicare & Medicaid Services (CMS). Pauline oversees and provides guidance in the development and implementation of innovative payment and delivery models related to advanced primary care and accountable care organizations, namely the Comprehensive Primary Care (CPC) and CPC Plus initiatives, the Pioneer ACO and Next Generation ACO Models, and the Comprehensive ESRD Care initiative.
