The PCPCC recently released a new report that supports primary care clinicians in their efforts to adopt a population health management (PHM) approach that effectively leverages health IT solutions and community partnerships in the larger 'medical neighborhood.' The webinar will include a presentation from Jaan Sidorov on the essential features of health IT-enabled PHM, and Robert Fortini's success at Bon Secours Virginia Medical Group, a Shared Savings ACO in Richmond, Virginia.

Robert Fortini, PNP

Chief Clinical Officer, Bon Secours Medical Group

Robert Fortini, PNP is the Chief Clinical Officer for Bon Secours Medical Group in Richmond Virginia. He is responsible for facilitating Provider adoption of EMR, coordinating clinical transformation to a Patient Centered Medical Home care delivery model, and facilitating participation in available Pay for Performance initiatives as well as Physician advocacy and affairs. He has extensive experience in Operations and Clinical Policy development and experience in workflow re-engineering and CQI in ambulatory care. Before coming to BSMG, Mr. Fortini served as the Chief Medical Affairs Officer at Queens Long Island Medical Group engaged in quality and HIT adoption. Richard Hodach, MD, MPH, PhD

Dr. Richard Hodach is the Chief Medical Officer of Phytel. Dr. Hodach has long been recognized as an advocate of integrating IT with the practice of medicine. Before joining Phytel, Dr. Hodach, a board-certified neurologist, was the senior vice president, chief medical officer at Matria Healthcare, where he provided strategic direction and clinical expertise in the development of evidence-based, patient-centric population health products.
