PCPCC's 2014 Western Regional Conference focused on identifying the challenges and tough issues that primary care and patient-centered medical home advocates will face in pursuit of delivery system reform, including payment reform, behavioral and mental health integration, health system consolidation, state health reform, and community partnerships. Our agenda includes a distinguished group of experts who will lead interactive panel discussions, as well as workshops that provide a deep dive on best practices and the "how-tos" of PCMH implementation.
