Team based care is an effective model for implementing patient self-management support within a practice. In this session you hear about practical steps and tools that are effective when utilized by primary care teams to engage patients in improving and managing their health.

Participants will be able to:

1. Define the components of goal setting and action plan development with patients as administered by a primary care team
2. Recognize ways to use the Electronic Health Record to support team work, team training and patient engagement
3. Explain a process for training team members to use communication skills and EHR tools to enhance patient self-management
