About the Program

With an overwhelming response to NCQA’s Patient-Centered Medical Home Recognition (PCMH) program, many are eager to learn more about the requirements and apply its criteria to improve quality. This advanced seminar examines the PCMH 2011 Standards further.  We will review the challenging standards, identify acceptable documentation for the elements, examine complex scoring rules and discuss how a practice can create and maintain a continuous quality improvement culture.  This course will also identify the meaningful use (MU) requirements embedded in the standards and how practices can receive credit for their MU achievements when seeking PCMH 2011 Recognition. 

The advanced program is intended for individuals who have substantial knowledge of the PCMH 2011 requirements and have applied these requirements within a practice setting.  Completion of this seminar is an eligibility requirement for NCQA’s new Patient-Centered Medical Home (PCMH) Content Expert Certification Program.   

At the conclusion of this educational program, the participant will be able to:

Identify and address the challenging NCQA Patient-Centered Medical Home (PCMH) standards.
Describe all phases of the PMCH submission and review process, and options and actions practices may chose to take after they have received their Recognition decisions.
Interpret the meaningful use criteria to obtain credit in the aligned elements.
Apply complex scoring rules when assessing practice readiness.
Establish and maintain a continuous quality improvement culture within a PCMH practice.

Who Should Attend

This interactive program is geared towards individuals with significant knowledge of the NCQA PCMH 2011 Standards and experience implementing those standards in a practice setting and who have attended the Facilitating Patient-Centered Medical Home Recognition seminar; and those who intend to obtain the individual Patient-Centered Medical Home (PCMH) Content Expert Certification credential.
