Beyond Policy: Implementing Care Coordination in Practice

The National Center for Medical Home Implementation (NCMHI) is hosting the first webinar in a three part series focusing on implementation and evaluation of pediatric care coordination. This webinar will outline clinical guidelines and identify strategies and resources to help facilitate the implementation of care coordination in practice. Faculty will provide real life examples of successful care coordination and highlight outcomes for patients and families on the receiving end of care coordination services.

This webinar will: 
1. Define care coordination and its role in meeting the comprehensive needs of pediatric patients and families. 
2. Identify clinical guidance, along with tools and resources, to facilitate care coordination implementation, as outlined in the 2014 American Academy of Pediatrics policy on pediatric care coordination. 
3. Provide examples of successfully implemented care coordination including practical strategies and specific outcomes from the perspective of families and clinicians.
