Community Integrated Health in Practices: Partnerships with Impact

Successful practice transformation requires the adoption of broad population health strategies. Community-based organizations can play an integral role in this arena.  As providers accept more risk under value-based payment models, there is a greater emphasis on managing chronic disease.  Evidence-based programs targeting persons with two or more chronic conditions have increasing importance to success with the introduction of new payment models.

Sally Lacy Sundar, MA

Director of Health Integration and Transformation, YMCA Association of Greater Seattle
Sally Lacy Sundar currently works as the Director of Health Integration and Transformation for the YMCA Association of Greater Seattle, where she directs the Association's work in formalizing operational and financial relationships with regional health care systems, insurers, employers, and other community organizations. She previously worked for the Centers for Disease Control and Prevention as a Public Health Associate providing technical assistance to the Richmond City Health District around collective-impact, cross-sectoral community health and policy initiatives. read more

Timothy McNeill, RN, MPH

Director, Clinical Integration, YMCA of the USA
Tim McNeill joins Y-USA as Director of Clinical Integration. In this role, Tim will help Ys position themselves as resources for navigating community health resources. He will also help guide Ys in the use of shared spaces with community health care providers. Tim is a registered nurse and has a master’s degree in Public Health from Eastern Virginia Medical School. He has served as a healthcare consultant for the past seven years, working with clients such as the Centers for Medicare and Medicaid Services and the U.S. Administration on Aging/Administration for Community Living. read more

Van is currently the Administrator for Population Health Management at UW Medicine providing direction and structure for carrying out quality and care commitments in value based contacts for Medicare Advantage, Commercial and Medicaid populations, building community partnerships and actualizing  the patient centered medical home in primary care to achieve better population health.
