ModRN Health aims to ensure patients and their families understand whats going on from the beginning of the health care process until the end. We don't want families to have several different people handling their care, but rather one nurse that is able to form a personal connection with them from the beginning and can moderate their overall care journey and their overall health records; providing a model that does not create more work for the providers nor the patient in order to be successful. Our nurses’ possess deep industry knowledge and expertise, a spirit of passion and an unstoppable drive and commitment for helping others. When combined with tools, such as our integrative, proactive care-management model the product is extremely powerful and allows us to offer the local healthcare community and patients alike, the solutions they are looking for; solutions that actually impact care quality and patient experience . Care management services can improve healthcare quality and safety through meaningful use of technology and customized preventative care, offering providers as well as health consumers, ways to reduce healthcare costs through improved organizational efficiency's, limiting unnecessary costs and waste, while driving engagement through customized education and empowerment strategies, measurable outcomes and best practices to optimize overall success.
