We are officially in #PCPCC2014 conference mode! The Annual Fall Conference is less than two weeks away - check out our exciting agenda . This year's conference is designed to offer practical "how to" support for practices, encourage local partnerships across communities to support patients and families, and provide an insider's view of public policy opportunities at the state and national level. Ness for her long-standing commitment to person-centered care, the Primary Care Community Leadership Award to Ted Wymyslo for being a remarkable community leader working to implement patient-centered medical homes in his state and the Patient-Centered Medical Home Practice Award to the Southeast Texas Medical Associates for being an extraordinary practice that has gone the extra mile to make medical homes a reality for all of their patients and families. Given the growing concern about sustainability payment to support PCMH and primary care innovations, we have asked Lisa Duksky-Watkins, MD, to share with us how states have led the charge on multi-payer collaborations. The Milbank Memorial Fund recently released a report that details the experiences of a voluntary group of state-based, multi-payer primary care transformation initiatives that are collaborations between payers, providers, employers and state officials. Patient-Centered Primary Care:
At the Heart of Value and Quality
November 12 - 14 | Washington, DC
Assistant S ecretary Kathy Greenlee was appointed by President Obama as Assistant Secretary for Aging at the U.S. Department of Health a Human Services and confirmed by the Senate in June 2009 where she also serves in the dual role of Administrator of the Administration for Community Living. She joins us to deliver the Opening Keynote on Wednesday, November 12th where she will discuss the importance of providing functional support to help those with health needs live independently in a home of their choosing, and help seniors and people with disabilities fully participate in their communities.
 Mark McClellan, MD, PhD, is Senior Fellow and Director of Health Care Innovation and Value Initiative at the Brookings Institution. The former Commissioner of the Food and Drug Administration and Administrator of the Centers for Medicare and Medicaid, Dr. McClellan's work focuses on promoting quality and value in patient centered health care. Dr. McClellan will discuss how the medical home has been vital to the success of the Accountable Care Organization during our Thursday morning Keynote. |

Family Medicine Announces Health is Primary Campaign

 This month, a coalition of organizations representing Family Medicine, including the American Academy of Family Physicians and the American Board of Family Medicine , announced Family Medicine for America’s Health and launched a communications campaign aimed at promoting primary care.
The goal of the campaign is to create an America where Health Is Primary – where doctors and patients work together, where everyone has access to primary care, and where prevention and health are as important as treating disease. Check out their website and stay in touch @HealthIsPrimary . |
