CDPHP is adding 70 Albany, New York-area primary care practices to its patient-centered medical home initiative, essentially doubling the size of the program.
The Albany-based health insurer started the program, which it calls its “enhanced primary care” initiative, in 2008 with three area practices. After two expansions, the program now includes 76 practices with 386 clinicians, and about 100,000 CDPHP members.
Phase Four, which kicks off Aug. 2, will add 70 practices and 274 clinicians. These practices see about 70,000 CDPHP members. CDPHP will give them each as stipend of $20,000—for a total investment of $1.4 million—and provide added support in the form of consultants, clinical coordinators and assistance with information technology.
As of year-end 2011, CDPHP had 364,275 members. This means nearly half of them will be in enhanced primary care settings.
