The NCQA PCMH Recognition program plays an integral role in practice transformation and improving the quality of health care. This advanced course is intended for individuals who have applied NCQA PCMH Recognition requirements in the practice setting. It examines the PCMH 2017 standards and the redesigned assessment process. It takes a hands-on approach to each PCMH Concept, and a deeper dive to explore the key characteristics of a successful medical home.
This course will discuss evidence to support the core and elective criteria. It will also focus on advanced topics such as the Distinction Modules, electronic Clinical Quality Measures, and how to receive autocredit. Attendees will participate in facilitated activities that demonstrate how earning elective credits builds upon the Core criteria of a Competency. Our faculty will incorporate topics on behavioral health integration, the importance of assessing the social determinants of patient health, engaging with patients and the community, and continuous quality improvement.
At the conclusion of the program, participants will have the knowledge they need to:
• Discuss vendor prevalidation, autocredit, electronic clinical quality measurement, and certified electronic reporting to facilitate recognition and annual reporting.
• Explain how elective criteria build upon core criteria within a competency.
• List the opportunities and challenges to succeed in the PCMH model of care.
• Determine a strategy for preparing a practice for the three (3) virtual check-ins and begin to create a transformation plan.
• Describe the types of evidence a practice can provide to meet the intent of a criterion, competency, and concept.
• Explain the role of PCMH Certified Content Experts in assisting practices through the Commit, Transform, and Succeed parts of the PCMH recognition.
This interactive program is geared towards individuals with significant knowledge of the NCQA PCMH Standards and experience implementing those standards in a practice setting and who have attended the Introduction to PCMH 2017 seminar; and those who intend to obtain the individual Patient-Centered Medical Home (PCMH) Content Expert Certification credential.
