This symposium, co-sponsored by Carefirst BlueCross BlueShield and the Patient-Centered Primary Care Collaborative, will showcase successes and lessons learned from the grantees of CareFirst's Patient-Centered Medical Home (PCMH) initiative, while providing a unique networking opportunity for PCMH providers, researchers, and other stakeholders. Free and open to the public, the program will feature a panel highlighting grantees' experiences and presentations from leading researchers discussing the future of PCMH and payment reform. Attendees also will hear an exciting announcement about future PCMH funding for safety net health centers. Continental breakfast and lunch will be provided.
