PCPCC Executive Member Monthly Briefing - Building a Bright Future for Primary Care: Comprehensive Primary Care Plus

Each month PCPCC Executive Members are invited to participate in exclusive monthly briefings to discuss internal and external developments, including strategic planning, public policy developments, and new program initiatives. Briefings occur monthly on the last Tuesday of the month from 4:00 PM – 5:00 PM ET.

Our April 2016 briefing will include a special presentation from the Centers for Medicare and Medicaid Services on the new Comprehensive Primary Care Plus (CPC+) Initiative, an advanced primary care medical home model and its largest investment in primary care transformation to date. CPC+ is a five-year regionally-based, multi-payer model that rewards value and quality by offering an innovative payment structure to support delivery of comprehensive primary care. The model will offer two tracks with different care delivery requirements and payment methodologies to meet the diverse needs of primary care practices. Members of the CPC+ model team will highlight the goals and innovative features of the CPC+ model design, followed by a brief discussion and Q&A.
