At a briefing on Jan. 30th, the Patient-Centered Primary Care Collaborative (PCPCC) will release a new report, The Patient-Centered Medical Home's Impact on Cost and Quality: Review of Evidence 2013-2014. This year’s Annual Review of the Evidence summarizes new results from primary care patient-centered medical home (PCMH) initiatives published from September 2013 through November 2014, since the publication of the previous Annual Review. Selected cost and utilization outcomes from a combination of peer-reviewed studies, state program evaluations, and industry publications are aggregated to present an overview of PCMH and primary care innovations happening across the country.

Industry experts will join a reactionary panel on Capitol Hill to discuss the publication as well as the latest PCMH evidence, innovative payment reform models, and perspectives from practicing physicians.

Christopher Koller is president of the Milbank Memorial Fund. The Fund is a 114 year operating foundation that improves population health by connecting leaders with the best information and experience. Before joining the Fund, he served the state of Rhode Island as the country’s first health insurance commissioner, an appointment he held from March of 2005 through June of 2013. Prior to AAFP, Dr. Henley was in private practice for 20 years in his hometown of Hope Mills, NC. He is a founding member of the Ambulatory Care Quality Alliance Steering Committee and serves on the steering committee of the Center for Payment Reform. Rahul Rajkumar, MD, JD, an internist, is the Acting Deputy Director at the Center for Medicare and Medicaid Innovation. Prior to arriving at CMS, Dr. Rajkumar was a consultant at McKinsey and Company, where he helped the senior management of leading hospitals and health care payers respond to national reform and the design and operational aspects of accountable care organizations. Dr. Rajkumar previously practiced hospital-based medicine at Brigham and Women's Hospital, where he completed his residency training in Internal Medicine.
