The NCQA’s Patient-Centered Medical Home Recognition (PCMH) program plays an integral role in practice transformation and the improvement of quality health care. This advanced seminar further examines the criteria of the PCMH 2014 Standards. With the newly updated standards, released in the spring of 2014, we are setting the bar higher for practices to strive for excellence. We will take a deeper dive into the standards by helping to identify the challenges one may face based on historical data and advisory panel feedback. In addition to the standards, we will review acceptable documentation for the elements, examine scoring rules and discuss how a practice can create and maintain a continuous quality improvement culture. This course will discuss the integration of behavioral health, the team based approach to care management, and also identify the meaningful use (MU) requirements embedded in the standards. Our faculty will indicate how practices can receive credit for their MU achievements when seeking PCMH 2014 Recognition.
The advanced program is intended for individuals who have substantial knowledge of the NCQA PCMH requirements and have applied these requirements within a practice setting. Completion of this seminar is an eligibility requirement for NCQA’s Patient-Centered Medical Home (PCMH) Content Expert Certification Program.*
*PLEASE NOTE: This course along with the Introduction to PCMH 2014(formerly titled Facilitating PCMH 2014) are the prerequisite courses to sit for the exam.
