The 2011 standards are more patient-centered with a significant emphasis on the planning, managing and coordinating care for patients. They have a greater emphasis on continuous quality improvement in primary care practices, and align with Federal initiatives of Meaningful Use.
Join us for a thorough review of the PCMH standards. We’ll practice scoring elements and examining documentation that supports an application towards becoming a recognized patient centered medical home with NCQA.
