More than 6,000 NCQA-Recognized Patient-Centered Medical Homes practices and over 32,000 recognized clinicians have proved the strong relevance of the 2011 PCMH Standards. The 2011 standards are more patient-centered with a significant emphasis on the planning, managing and coordinating care for patients. They have a greater emphasis on continuous quality improvement in primary care practices, and align with Federal initiatives of Meaningful Use.

Join us for a thorough review of the PCMH standards. We’ll practice scoring elements and examining documentation that supports an application towards becoming a recognized patient centered medical home with NCQA.

The purpose of this educational program is to help participants:

  • Examine the essential characteristics of a recognized patient-centered medical home
  • Identify the measurement and documentation criteria for each of NCQA’s updated requirements
  • Facilitate the NCQA survey and evaluation process

Seats are limited. Early registration is highly recommended. This seminar tends to sell out quickly.
