PCPCC Cancer Care Webinar Series: Working with Care Teams During Treatment

The PCPCC, in partnership with the American Cancer Society will be launching a three part webinar series in December 2013 that focuses on how patient-centered medical home providers and practices can support patient health and caregiver support after a cancer diagnosis, during treatment, and post-treatment. Expert speakers will discuss how to support patients throughout the care experience including initial diagnosis, active treatment and survivorship.

The project also includes a collection of helpful guides and checklists to encourage strong patient-provider communication and help identify the different roles and responsibilities of the primary care team and the oncology team.

This webinar will focus on the relationship and partnership between patients, families, the medical home care team, and specialists as patients engage in active, ongoing cancer treatment.

Carmelita Escalante, MD, FACP is a Professor and the Department Chair of the General Internal Medicine Department at MD Anderson Cancer Center in Houston, Texas. Dr. Escalante's major accomplishments include developing the first Cancer-Related Fatigue Clinic in the nation, building a number of clinical programs, and developing a research program in general internal medicine with her department leading all others in the Division of Internal Medicine with regards to funding. Lyle Green is the Associate Vice President for Physician Relations at The University of Texas MD Anderson Cancer Center, where he is responsible for providing leadership for enhancing physician access, communications and engagement strategies. Lyle provided the initial vision for and led the design, development and implementation of the myMDAnderson physician portal, which provides MD Anderson’s referring physicians with a highly customized online referral process along with secure, web-based access to their referred patient’s medical record information. Today, over 13,000 physicians in 113 countries use the physician portal to support clinical communications and to facilitate coordination of care for their cancer patients. read more

Manager, Medical Home Initiatives, American Academy of Pediatrics

Dana Bright, LSW, MSW is currently the Manager, Medical Home Initiatives at the American Academy of Pediatrics in Elk Grove Village, IL. In this role, she supervises medical home initiatives including pediatric residency and other educational activities, medical home chapter champions and the interface between primary and subspecialty care. She also provides technical assistance to pediatricians, other health professionals and key stakeholders on the integration of medical home and quality improvement concepts in practice in order to improve access to health care services for all children and youth. read more
