Missouri

The Missouri Health Improvement Act of 2007 included a provision that the Department of Social Services should create health improvement plans for all participants in MO HealthNet to include coordinated care plans and be enrolled in a health care home. The Missouri Foundation for Health launched the Advancing Chronic Care through Excellence in Systems and Support (ACCESS) program in 2010, designed to improve the delivery of chronic illness care. ACCESS works with Federally Qualified Health Centers (FQHC), local Health Departments, hospitals, and academic health centers through a learning collaborative model to improve team care and care coordination.  In 2011, Missouri was awarded the first approved State Plan Amendment (SPA) to implement their Section 2703 Health Homes program. To accelerate the transformation to a Health Home model, the Missouri Foundation for Health and the Healthcare Foundation of Greater Kansas City funded an 18-month Learning Collaborative for behavioral health and primary care teams.

Coordinating All Resources Effectively (CARE) - Missouri

Children’s National Health System has been named a recipient of a Center for Medicare & Medicaid Innovation (CMMI) award to improve health care delivery for children with complex medical needs.  The grant, aimed at advancing care coordination models, was awarded to the Children’s Hospital Association (CHA) and 10 pediatric hospital partners.

Enhanced Personal Health Care Program - Missouri

Anthem Blue Cross and Blue Shield in Missouri announced that SSM Health Care will join its patient–centered care program, Enhanced Personal Health Care. This initiative will create an accountable care model to help Anthem members with chronic conditions improve their overall health through coordination of care and sharing of information. 

Missouri Health Homes Program

Missouri has two approved 2703 Health Home State Plan Amendments. The first State Plan Amendment was approved in October of 2011 and covers individuals with a Serious and Persistent Mental Illness (SPMI), mental health or substance abuse disorder plus a chronic condition, Mental Health or Substance Abuse Disorder plus tobacco use
