P2 Collaborative

The P2 Collaborative is integrating the medical home model into a new regional planning initiative for Western New York. It is one of 16 communities in the country selected by the Robert Wood Johnson Foundation to be an Aligning Forces for Quality (AF4Q) community. P2 Collaborative has been working to establish a community based approach to providing quality improvement, change management and technical assistance resources to assist health care practices in achieving Patient-Centered Medical Home.  The P2 Collaborative uses a Community Extender Model to serve the practices in the region. It serves as the coordinator of services to primary care practices throughout Western New York, subcontracting with Community Based Extenders that cover the eight counties in the region, who in turn hired locally based quality improvement resources, called Practice Enhancement Associates (PEAs).

As coordinator, convener, facilitator and conduit for funding, the P2 Collaborative is in a position to help all levels of the healthcare delivery system improve, coordinating physicians, practices, hospitals, consumers, businesses, payers and local and state government to gain significant buy-in through out the transformation process.  Western New York Quality Measurement Collaborative (WNYQMC) was formed by the three full-service health plans BlueCross/BlueShield of Western New York, Independent Health and Univera Healthcare as the governance organization for a New York State Department of Health (NYSDOH) grant to implement a pay-for-performance program targeting improved provider performance in acute heart attack treatment, diabetes management, antibiotic use, and infection control. |The P2 Collaborative of Western NY is composed of several community health centers/clinics in Buffalo and Niagara Falls that are considering interventions aimed at improving the efficiency and quality of care for their diabetic Medicaid population.
