The Collaborative Accountable Care program links doctors, hospitals and a single health plan together with the goal of reducing health care costs by improving patients' health. The principles of the patient-centered medical home are the foundation of Cigna's collaborative accountable care initiatives. Cigna then builds on that foundation with a strong focus on collaboration and communication with medical organizations.
Care coordinators work closely with Cigna's case managers and help their patients access Cigna's clinical support programs, such as chronic condition management (diabetes, heart disease) and lifestyle management programs (weight, stress, tobacco). These programs focus on expanding patient access to health care, improving care coordination, and achieving the “triple aim” of improved health outcomes (quality), affordability and patient satisfaction.
Cigna will compensate Colorado Springs Health Partners for the medical and care coordination services they provide. Additionally, they will be rewarded through a “pay for value” structure for meeting targets for improving quality and lowering medical costs.
