Individuals with disabilities dually eligible for Medicare and Medicaid are at higher risk for poor health outcomes when compared to individuals without disabilities.[1]  This conversation, in collaboration with ICS, will provide a platform for questions to be answered on how health plans and providers can improve health outcomes for this population by addressing gaps in care coordination that often disrupt continuity of care and lead to fragmentation of care.
