The collaborative is the country’s largest program dedicated to bringing together family medicine, internal medicine, and community health centers to achieve medical home recognition from the National Committee for Quality Assurance (NCQA), practice transformation, and improve patient health outcomes, especially those with chronic diseases. The collaborative began in 2010 as a faculty-led, family medicine residency collaborative and grew to add internal medicine and community health centers in 2011. The program is supported by grant funding from the PAFP and was recently selected by the Centers for Disease Control and Prevention (CDC) as a model learning method to evaluate and study over the next 30 month period. Webinar participants will learn the key components of the collaborative model and methods for replicating the model in other settings.
