President & CEO, Institute for Patient and Family-Centered Care
Beverley H. Johnson serves as the President and CEO of the Institute for Patient and Family-Centered Care and has over 25 years of experience in organizational development and management. She has worked as a health professional providing direct care in hospitals, managing a hospital unit, and teaching. She has served as a trustee of a hospital, national health care organization, and a school board. In these trustee roles, she has chaired and served on strategic planning committees and had fiduciary responsibility for nonprofit organizations. She has provided technical assistance and consultation to over 250 hospitals across the United States and Canada. She has published widely on patient- and family-centered issues and strategies. read more
Hala Durrah is a national speaker, patient family centered care consultant and advocate. Her professional background includes event and meeting management, non-profit work, development, and writing. She earned her Bachelor’s degree in International Affairs and Master’s degree in Tourism Administration, both from the George Washington University in Washington, D.C. Her current work in patient family centered care stems from her experiences as a mother of a chronically ill child who has undergone two liver transplants and a bone marrow transplant. read more
Senior Policy and Program Specialist, Institute for Patient- and Family-Centered Care
Marie provides consultation to hospitals and primary care and ambulatory care practices, assists with the development of resource materials, co-authors publications, and serves as a planner and faculty member for Institute conferences, seminars, and webinars. Currently, Marie is project co-director for a Robert Wood Johnson Foundation funded project that provides training and technical assistance to teams from their Aligning Forces for Quality Initiative. Since joining the Institute in 1996, Marie has worked on several international and national quality improvement initiatives focusing on newborn intensive care, maternity and normal newborn care, and primary care for children and adults with chronic conditions. Marie lives in Connecticut with her three sons, one of whom required early intervention for his first six years.

Senior Policy and Program Specialist, Institute for Patient- and Family-Centered Care
Mary Minniti is a Certified Professional in Healthcare Quality and has over 30 years experience in the field. She joined the Institute for Patient- and Family-Centered Care in 2011 and works as a Senior Policy and Program Specialist. She was lead author on a Robert Wood Johnson Foundation funded report, Individual and Family Engagement in the Medicaid Population: Emerging Best Practices and Recommendations and IPFCC’s publication: Essential Allies Patient, Resident, and Family Advisors: A Guide for Staff Liaisons. read more
National Health Officer, YMCA of the USA (Y-YMCA)
Matt Longjohn, M.D., M.P.H., is the National Health Officer at the YMCA of the USA (Y-USA). In that capacity, he oversees a team of ~30 content experts and technical advisors charged with developing evidence-based policies, programs and practices; and then to bring these resources to national scale through the Y’s ~900 non-profit organizations, 2700 facilities, and 10,000 program sites. read more
Sara Guastello is Director of Knowledge Management for Planetree. In this role, she draws on current research and observational field work in developing a range of educational and implementation resources to guide healthcare organizations in their endeavors to deliver patient-centered care. Much of her work concentrates on the quality of human interactions that occur within healthcare settings, the importance of connecting healthcare personnel to the purpose and meaning of their work, and practical strategies for recasting patients and family members as true partners in care. read more
Technical Advisor, Chronic Disease Prevention Programs, YMCA of the USA
Suzi Montasir currently provides technical assistance and support to YMCAs across the county implementing the YMCA’s Diabetes Prevention Program. In addition, she works specifically with Ys delivering programs funded by the CDC Community Transformation Grant (CTG) and Racial and Ethnic Approaches to Community Health (REACH) initiatives with a focus on building linkages between health care providers and community-based organizations in African American and Hispanic/Latino communities. As health care evolves to become more patient-centered in both care delivery and payment, the PCPCC’s Support and Alignment Network (SAN) aims to assist patient, family, and clinician partners in their collaborative efforts towards quality improvement and practice transformation.
New to our conference this year, we are offering a special training for clinicians and their patient, family and/or community partners. This training provides the opportunity to learn alongside your patient partners, identifying “best practices” and strategies for quality improvement and care redesign. You will also learn the best ways to integrate patient- and family-centered care principles into care design throughout your organization, how to effectively perform in your respective roles, and more!

Overview
- Wednesday, Nov. 9, 5:00 p.m. to 6:00 p.m. ET– Patient and Family Partner Reception
- Thursday, Nov. 10, 7:30 a.m. to 6:30 p.m. ET– PCPCC Annual Fall Conference.
- Friday, Nov. 11, 8:00 a.m. to 4:00 p.m. ET– The Transforming Clinical Practice Initiative, backed by CMS, broadens the window of opportunity for providers to include their patients/family caregivers in quality improvement activities. This day will be dedicated to providing partners with the knowledge, tools and skills required to drive sustainable clinical transformation in ambulatory care settings. You will also work with expert faculty and experienced clinician-partners to develop your own plan for practice improvement based on the TCPI Change Package and your learning throughout the track.

General conference registrants are welcome to participate in the patient, family and clinician partners training at no additional charge. Individuals interesting in only attending the patient and clinician partner training should still register for the general conference for entry.
Scholarship opportunities are available for patient adviors, caregivers, clincians, and community partners to attend!
