As health care evolves to become more patient-centered in both care delivery and payment, the PCPCC’s Support and Alignment Network (SAN) aims to assist patient, family, and clinician partners in their collaborative efforts towards quality improvement and practice transformation.

New to our conference this year, we are offering a special training for clinicians and their patient, family and/or community partners. This training provides the opportunity to learn alongside your patient partners, identifying “best practices” and strategies for quality improvement and care redesign. You will also learn the best ways to integrate patient- and family-centered care principles into care design throughout your organization, how to effectively perform in your respective roles, and more!

Overview

  • Wednesday, Nov. 9, 5:00 p.m. to 6:00 p.m. ET– Patient and Family Partner Reception
  • Thursday, Nov. 10, 7:30 a.m. to 6:30 p.m. ET–  PCPCC Annual Fall Conference.
  • Friday, Nov. 11, 8:00 a.m. to 4:00 p.m. ET– The Transforming Clinical Practice Initiative, backed by CMS, broadens the window of opportunity for providers to include their patients/family caregivers in quality improvement activities. This day will be dedicated to providing partners with the knowledge, tools and skills required to drive sustainable clinical transformation in ambulatory care settings. You will also work with expert faculty and experienced clinician-partners to develop your own plan for practice improvement based on the TCPI Change Package and your learning throughout the track.

Who Should Attend?

Patient and Family Advisors and Leaders
Clinicians (i.e. Physicians, Nurses, Social Workers, Therapists, Pharmacists, Child Life Specialists, and other clinical staff)
Health Care Administrative Leaders
Quality, Safety, and Patient Experience Personnel

General conference registrants are welcome to participate in the patient, family and clinician partners training at no additional charge. Individuals interesting in only attending the patient and clinician partner training should still register for the general conference for entry. 

Scholarship opportunities are available for patient adviors, caregivers, clincians, and community partners to attend!
